A report by the Alaska Division of Forestry (DOF) noted that four firefighters survived a plane crash near the western Alaskan village of Aniak. The DOF owned the plane that crashed into the Kuskokwim River on takeoff. Government officials are investigating the crash for mechanical failure.

A DOF report said that there were four firefighters on board along with a pilot. All on the plane suffered injuries from the crash. The seriousness of the injuries was not released. Alaska State Troopers and local Emergency Services personnel responded to the crash. DOF notified The National Transportation Safety Board and Federal Aviation Administration.
The plane, an Aero Commander 500 Shrike, had emergency firefighters from two western Alaska villages to Soldotna onboard. The firefighters were going to extinguish wildfires in the Kodiak Forestry station. The plane crashed almost immediately after takeoff.
